Who we are

A collaborative community health organization.

Unity Community Health Center is a new not-for-profit created to improve equitable access to health, wellness and community supports in Ontario.

We bring together health-system navigation, primary-care partnerships, mental health and wellness, complex case management, healthy aging, trauma-informed services, prevention and outreach. Instead of duplicating existing services, we build referral pathways with clinics, hospitals, settlement and community agencies so care is coordinated around the person.

Vision

A healthier, more inclusive and equitable Peel Region where everyone can access coordinated, understandable care.

Mission

Connect underserved individuals and families with accessible, culturally responsive and trauma-informed supports.

Why Peel Region

One of Canada's largest and most diverse communities.

Peel's scale, diversity and growth make accessible, culturally responsive and well-coordinated services a necessity.
