About Unity
A new, federally incorporated not-for-profit building a trusted point of connection between Peel's communities and the health and social services that support them.

Coordinated care, built through partnership.
Unity Community Health Center was created to improve equitable access to health, wellness and community supports in Ontario.
Our model brings together health-system navigation, primary-care partnerships, mental health and wellness supports, complex case management, healthy aging, trauma-informed services, prevention, education and community outreach.
We are developing Unity as a collaborative community health organization. Rather than duplicating existing services, we build strong referral pathways and partnerships with primary-care providers, hospitals, community agencies, settlement organizations, mental-health services, seniors organizations, GBV and survivor-support services, and public institutions serving Ontario's residents.
A healthier, more inclusive and equitable Peel Region where every person can access coordinated, culturally responsive and understandable health and community services.
To improve health and well-being by connecting underserved individuals and families with accessible, integrated, culturally responsive and trauma-informed supports, while strengthening collaboration across the community.
What guides our work.
Equity is built in, not added on.
Health outcomes are shaped by more than medical care. Language, culture, age, disability, income, migration experience, discrimination, trauma and gender all play a part. We design programs around accessibility, community participation, cultural responsiveness, confidentiality, informed choice and respect for lived experience.
